The Peer Trainer will develop and offer a series of workshops designed to help peer specialists learn about different specialties in peer work and practice the skills that different roles would require. This would help better prepare those who are interested in working with people who are experiencing mental health crises in a variety of settings such as: mobile crisis response teams; in hospitals; in crisis respite residences; street outreach; or on Intensive Mobile Treatment or Assertive Community Treatment Teams (IMT/ACT), and Intensive and Sustained Engagement Team (INSET).

In addition to attending team meetings, Peer Specialists work directly with other team members to provide support and assistance to program participants. Under the supervision of the OnTrackNY Team Leader, they provide direct peer support services to program participants in any setting that would be helpful – at the OnTrackNY program site, in the community, or in the hospital.

The Peer Specialist will work with high risk of recidivism individuals who have been diverted from Manhattan Criminal Court, facilitating groups, providing individualized rehabilitation support and connecting participants to substance use services, cognitive‑behavioral groups, peer support and employment resources within a recovery‑oriented, self‑determination and public safety framework.

JOB DEFINITION:

The Peer Specialist functions as a member of an interdisciplinary care coordinator team to assist with the outreach, support and coordination of care for those eligible for and/or served by the Forensic ACT Team, including chronically medically ill individuals with co-occurring mental illness and/or substance abuse disorders. Peer Specialist will serve as a role model and/or health coach for clients.

JOB DEFINITION: The Certified Peer Specialist functions as a member of the Intensive Crisis Stabilization Center to assist with the outreach, support and coordination of care for those eligible Nassau County residents Peer Specialist will serve as a role model and/or health coach for clients.

EDUCATIONAL REQUIREMENTS: High School Diploma or equivalent required.
Peer certification (NYCPS, CRPA, or FPA) required.

The Career Coach provides personalized employment related support including job/internship preparation and search, job development, placement, and job retention coaching to HTH participants. Through a customized array of services delivered both on site and in the field the Career Coach provides support and skill development toward success and satisfaction as a trainee in the HTH classroom, on an internship, and while engaging and sustaining employment.

The Program Manager is responsible for the coordination of peer initiatives within the Agency, including those at Riverview Apartments and the Wishing Wellness Center.
