As an Outreach Certified Peer within a CCBHC (Certified Community Behavioral Health Clinic) model, you will play a critical role in engaging individuals who are disconnected from services or at risk of falling through the cracks. Drawing from your lived experience, you will meet people where they are — physically, emotionally, and spiritually — to offer support, connection, and the belief that recovery is possible. Your efforts will help bring individuals into care and guide them along their recovery journey through trust, compassion, and empowerment.

The Peer Care Transition Specialist supports individuals transitioning from hospital and behavioral health settings back into the community. Using lived experience and a recovery-oriented, trauma-informed approach, this role collaborates with healthcare providers and community partners to promote continuity of care, reduce barriers to treatment, and support recovery, wellness, and independence. Responsibilities also include supporting Social Care Network (SCN) initiatives through screenings, referrals, and coordination of Health-Related Social Needs (HRSN) services.

OnTrackNY is an innovative treatment program that serves adolescents and young adults who are within 2 years of experiencing the onset of non-affective psychosis. The Peer Specialist works closely with all members of the small multidisciplinary team in an integrated way providing coordinated engagement and support in the earliest stages of illness. The Peer Specialist is a resource for the client and their family, coordinating meetings and working with the client within a shared decision-making framework.

We are currently seeking a Peer Specialist for our Intensive Services Unit (ISU). Rapidly after release into the program, the Peer Specialist will assist the Clinical team with conducting Supervision Introduction/Orientation meetings with new participants to review supervision conditions required under the supervision plan. Additionally, the Peer Specialist will work alongside a clinician, together they will assist program participants and focus on delivering proactive field-based direct services and limited coverage at court.

Build relationships with clients and their networks of support in order to provide hope and support the person’s recovery.
Assist clients with navigating the service systems, including behavioral and medical health, criminal justice, shelter system, entitlements and transportation.
Complete progress notes in the electronic health record within a timely manner for all services provided. Complete other assessments, such as crisis relapse prevention plans.
Work with external stakeholders – such as hospitals, shelters, jails, primary care – to achieve positive client outcomes.

The program helps individuals achieve recovery goals through Psychosocial Rehabilitation and Peer Support. Services address daily living skills, finances, housing, education, employment, relationships, social connection, behavioral and primary healthcare access, personal recovery, resilience, and full community integration. Certified Peer Specialists provide Peer Support and direct care.

The Peer Counselor / CRPA provides non-clinical peer-support services.
Unless separately licensed or credentialed and specifically authorized by AllHealth, you may not independently:
Diagnose substance use, psychiatric, or medical conditions;
Conduct clinical assessments;
Provide psychotherapy;
Make medical or psychiatric treatment decisions;
Prescribe or recommend medications;
Independently establish or modify a clinical treatment plan; or
Perform services requiring a professional license or credential you do not possess.

Serve as a role model for recipients and educate recipients about self-help techniques and self-help group process; teach them effective coping strategies, sometimes based on personal experience. Preferred qualifications: lived mental health experience from first-hand, personal knowledge, understanding gained from navigating and receiving services and/or supports as an element of their recovery; experience as a peer advocate in the mental health system.
